چکیده

despite best efforts, concern remains about potential variations in rectal cancer treatment and outcomes. The System Performance Initiative at the Canadian Partnership Against Cancer regularly reports on standardized indicators at a national level to help identify opportunities for pan-Canadian system improvements. Recognizing that surgical resection is generally recommended as first-line treatment for nonmetastatic disease, the first indicator presented in this rectal cancer surgery–focused report is the rectal cancer resection rate. The second indicator is the rate of crm positivity among patients undergoing rectal cancer resection. A better understanding of the data can help to identify best practices, which in turn can inform quality improvements.

Prognostic significance of the circumferential resection margin following total mesorectal excision for rectal cancer.

BACKGROUND Knowledge of prognostic factors following resection of rectal cancer may be used in the selection of patients for adjuvant therapy. This study examined the prognostic impact of the circumferential resection margin on local recurrence, distant metastasis and survival rates. METHODS A national population-based rectal cancer registry included all 3319 new patients from November 1993 t...

Effect of the plane of surgery achieved on local recurrence in patients with operable rectal cancer: a prospective study using data from the MRC CR07 and NCIC-CTG CO16 randomised clinical trial

BACKGROUND Local recurrence rates in operable rectal cancer are improved by radiotherapy (with or without chemotherapy) and surgical techniques such as total mesorectal excision. However, the contributions of surgery and radiotherapy to outcomes are unclear. We assessed the effect of the involvement of the circumferential resection margin and the plane of surgery achieved. METHODS In this pro...

Reproducibility of depth of extramural tumor spread and distance to circumferential resection margin at rectal MRI: enhancement of clinical guidelines for neoadjuvant therapy.

OBJECTIVE The purpose of this study was to evaluate the reproducibility of measurements of minimal distance from an invasive tumor to the anticipated circumferential resection margin in prediction of depth of extramural tumor spread in patients with rectal cancer. MATERIALS AND METHODS Images from 168 consecutive pelvic MRI examinations of patients with rectal cancer were evaluated by radiolo...

Oncologic Outcomes and Risk Factors for Recurrence after Tumor-specific Mesorectal Excision of Rectal Cancer: 782 Cases

PURPOSE The aim of this study was to analyze the oncologic outcomes and the risk factors for recurrence after a tumor-specific mesorectal excision (TSME) of resectable rectal cancer in a single institution. METHODS A total of 782 patients who underwent a TSME for resectable rectal cancer between February 1995 and December 2005 were enrolled retrospectively. Oncologic outcomes included 5-year ...
